Know how well your child's ADHD treatment is working at school — and when it may need to change. Here's how it works:

Enter your child's info, teacher, and optional clinician — about 5 minutes.

Teacher is contacted
Your teacher receives an intro email and recurring rating form requests.

Clinician is notified
Your child's clinician — if you add one — receives an intro email and gets results automatically.

You receive results
After each form is submitted, you receive a clear results summary by email — including graphs showing trends over the school year.
